Stretching & Joint Mobility
 
Mobility can be affected by many factors. Our body responds to the forces that are placed upon it. Most tissues require movement to maintain their structural integrity and health.
Mobility is a major factor in quality of life and human function.
According to Wolf’s law, the human body will remodel itself based upon the demands that are placed upon it. Our bodies are alive and respond to the internal and external environment. Soft tissue such as cartilage, ligaments, tendons, muscles and nerves are highly adaptable to the amount of movement and stress they receive. In fact soft tissues such as these are shaped specially in order to tolerate the demands the human body must encounter in daily life. If these tissues experience too little stress and movement it will become weakened and less pliable. If the tissue experiences stresses and movement forces that are too high they will be damaged. If we get the right amount of movement and stresses to these tissues they will become stronger and will adapt to prepare to respond to encounter these forces at a later time. The body prepares itself to make a difficult task easier the next time it has to encounter that task.
 
 
Nerves are also surrounded by connective tissue. This connective tissue can become tight and create tension on our nerves. This will ultimately lead to pain and potential lack of oxygen to the nerve which can lead to numbness and tingling sensations as well. Stretching a nerve is very specific. Many people think they are stretching a muscle when they are actually stretching a nerve. If they are stretching a nerve like a muscle they may cause further damage to the nerve.
 
Obviously stretching is not as straight forward as it seems. At Revolution Physical therapy we understand how to specifically stretch each type of tissue and will identify the appropriate type of stretching the patient needs in order to keep their connective tissue mobile.
 
Mobilization
 
Creep is the phenomenon which describes how a tissue deforms from a load. If a high force is applied quickly to a tissue, the tissue rebounds quickly and back to its original shape. If the load or force is applied at a low intensity over an extended period of time permanent deformation will occur. It takes about 6-8 weeks to see a structural change in muscle tissue length.
 
Different tissues have different properties and need specific types of mobilization. Muscle tissue needs to be stretched and elongated to the end range position and held. The joint capsule and ligaments can also get tight and, depending on the joint, will respond to either static stretching, rhythmic oscillations or even manipulation.
 
Manipulation takes the tissue past its end range which often allows air to flow out of the joint creating and audible “pop”. This type of mobilization in physical therapy is termed a grade 5 mobilization due to territorial legal issues between chiropractors and physical therapists. Grade 5 mobilizations and manipulations should only be done by a trained professional.

Total joint replacement can be performed on any joints of the body, including the hip, knee, ankle, foot, shoulder, elbow, wrist, and fingers. Combined, there are over half a million total hip replacements (THRs) and total knee replacements (TKRs) performed in the United States yearly.
